Procedures for requesting disclosure, etc. of retained personal data

We will respond to requests for disclosure, etc. (notification of purpose of use, disclosure, correction, addition, or deletion of content, suspension of use, elimination, suspension of provision to a third party, and disclosure of records of provision to a third party) of personal information held by us. Please contact the "Inquiry Desk" below to make a request for disclosure, etc. We will send you a Disclosure Request Form. Please check the contents of the Request for Disclosure Form, fill in the form according to the items, and return it to us. When making a request, you may be asked to present documents that prove your identity or that of your representative. We will respond to your request for disclosure, etc. by telephone or in writing.
If you request "notification of purpose of use" or "disclosure," please enclose a postal money order or stamps for 500 yen with your request for disclosure, etc. (Please note that the fee and the registered mail that will send you the response will be charged to your account. (This will be used as the fee and registered postage to send the response.)
When making a request for disclosure, etc., you may be asked to submit the following documents to verify that the person making the request is the person in question or a representative.

(1) If the person making the request for disclosure, etc. is the principal, please attach one of the following items

  • Copy of driver's license, passport, personal number card (only the side showing name and photo), etc.
  • Copy of national qualification card, health insurance card, pension book, resident card (within 3 months, without personal number)
  • A copy of certificate of residence for foreign residents

(2) When the person requesting disclosure, etc. is a representative

  • One of the above documents and a letter of attorney from the principal (Please submit a seal by the principal and a certificate of seal registration of the relevant seal. If the proxy is a legal representative such as a person with parental authority, please submit a document that shows the relationship with the person in question. If the proxy is a lawyer or other qualified person, please state his/her occupation and registration number.)

Cookies are used for session management purposes, but no personal information is obtained.
